The Case Manager plays a vital role in delivering client-centered services that support patients across the continuum of care. This position partners with patients and their families to coordinate healthcare and connect them with appropriate home- and community-based services. We are seeking a skilled and patient-focused Case Manager (RN) to join our growing team.

Key Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans
• Coordinate care across providers and service settings
• Advocate for patients and their families to ensure access to necessary resources
• Provide education to patients, caregivers, and support systems
• Monitor patient progress and adjust care plans as needed
• Maintain accurate, timely, and compliant documentation
• Deliver age-appropriate care based on developmental needs

Required Qualifications

Active, unrestricted Registered Nurse (RN) license in the State of Georgia (must be maintained) 3+ years of clinical experience 1+ year of case management experience preferred Preferred Skills & Abilities Strong computer proficiency, including Excel Experience with Electronic Medical Records (EMR) systems (Athena experience a plus) Excellent communication, organization, and problem-solving skills Why Join Millennium Physician Group? Compensation Range

$73,343.00 to $110,014.00 The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
