Full-time, 40 hour/week, Day shift Registered Nurse Case Manager position at Lincoln Hospital (MaineHealth). The role collaborates in case finding/screening, assessment, planning, implementation, evaluation, monitoring, service referrals, medication reconciliation, risk call backs and documentation of patient's discharge transition needs. Requires graduate of accredited School of Nursing, two years of experience, acute care experience and knowledge of discharge planning, home care and case management. BSN preferred. Certification or education in Case Management preferred.
