We're seeking a dedicated Hospice Registered Nurse (RN) Case Manager to join our team for a full-time position (40 hours per week).
Job Summary
As a Hospice RN Case Manager, you'll play a crucial role in planning, organizing, and directing hospice care. This position requires a blend of nursing expertise, community health education, and a deep commitment to end-of-life care. You'll be the cornerstone in building a bridge between our organization's resources and the needs of patients and families in their homes and communities.

Qualifications

- 1-2 years recent acute care experience (preferred)

- Empathy and comfort with end-of-life care
Work Environment
- Mix of facility and home-based care
- Average caseload: 12-15 patients
- Monday – Friday, 8am-5pm (some on-call and weekends)
- Centralized territory assignment
