Position Overview Safe Life Home Health Care, LLC is seeking a skilled Home Health RN Case Manager with a minimum of 1 year home health experience and proficiency in WellSky Kinnser EMR to join our 4-star team. Prioritize patient health and help prevent hospitalization while providing exceptional care in the comfort of patients' homes. Safe Life utilizes Andy AI , which assists our clinicians with OASIS completion and submission within 24 hours. Responsibilities Provide strong clinical skills, excellent communication abilities, and a passion for patient-centered care Understands and adheres to established policies and procedures Conduct comprehensive assessments of patients' health status and needs in their home environment Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient’s physical or emotional condition Develop and implement individualized care plans based on patient assessments and medical history Administer medications and treatments as prescribed, ensuring adherence to safety protocols Monitor patients’ progress, documenting changes in condition and response to treatment Provide education to patients and families/significant others regarding disease process and management, medication regimen, pain management, fall prevention, infection control, proper diet / exercise, when to contact pharmacy / physician / SN and when to seek emergency care. Collaborate with interdisciplinary teams to coordinate care, including discharge planning and referrals to other services as needed Maintain accurate medical records and documentation in compliance with legal and regulatory standards Respond promptly to patient inquiries or concerns, providing support and guidance as necessary Attends staff meetings, patient care conferences and in-services as scheduled Qualification Valid Illinois RN license Valid CPR Certification Valid Drivers License Valid Auto Insurance Minimum 1 year home health experience Proficiency in WellSky Kinnser EMR Strong clinical assessment and communication skills Compassionate and patient-centered approach Advantages Competitive compensation package with overtime Health, dental, and vision insurance Tuition Reimbursement Continuing education opportunities Bonus opportunities Retention bonus Yearly bonus Pay Per Visit (Based on visit type) Medical Specialty Home Health Expected Hours 15 – 40 visits per week Experience Minimum 1 year home health experience License / Certification IL RN license / CPR
