Main function
A caregiver who assumes responsibility and accountability for a group of patients under the supervision/direction of a registered nurse. Reports to the Director of Department to which assigned.

Duties and
Responsibilities
Performs ongoing patient assessments, collecting physiologic, psychological, and cognitive health data to support quality patient care. Provides direct patient care under the supervision and direction of a Registered Nurse, following established plans of care. Recognizes changes in patient condition and promptly communicates significant findings to the RN in a clear and organized manner.
Implements nursing interventions safely and competently, prioritizing patient needs and delivering care consistently. Demonstrates proficiency in nursing skills, medication administration, and clinical procedures while adhering to organizational policies and standards. Documents patient care, interventions, and responses accurately and promptly in accordance with legal and professional requirements.
Supports patient and family education by identifying learning needs, reinforcing teaching plans, and promoting health literacy. Maintains patient confidentiality, upholds patient rights, and follows safety, infection control, risk management, and emergency response protocols. Exhibits professionalism through accountability, effective communication, teamwork, reliability, and positive working relationships with patients and healthcare team members.
Participates in ongoing professional development, mandatory education, competency requirements, customer service initiatives, and organizational goals to promote high-quality patient care.
Education
Graduate of a Practical Nursing Program registered by the New York State Education or by the licensing authority of another state.
Cardiopulmonary resuscitation (cpr)
Requirements
Basic Life Support (BLS) required within 90 days of hire date. Advance Cardiac Life Support (ACLS) is required in critical care units.

Appreciable physical effort required. Moving, lifting and assisting patients whose size and physical abilities can vary greatly. Some lifting, reaching and stooping. Required to be on feet much of the time.
