The Full Time RN Case Manager provides nursing care with special emphasis on control of pain and other symptoms to the hospice patient. Implements physician plan of care and establishes a nursing plan of care based upon nursing process. Trains, supervises and supports the family in provision of care to the patient.
Job Functions
Assess the physical and psychological status of the patient. Reviews past and present medical history of the patient. Perceptive in planning and problem solving. Provides emotional support to caregivers. Able to work independently and make critical decisions based on available patient information. Performs ongoing assessment of patients conditions, physical needs and needs of family for support in giving care. Maintains ongoing communication with prescribing physician for changes in medication and treatments as needed. Documents hospice patients condition and nursing interventions on clinical record.
Qualifications
Licensed as a Registered Nurse in State of CT Hospice experience preferred Knowledge of Medicare and Medicaid requirements outlined in the laws of licensure in the state (s) which he/she practices. Ability to relate and treat terminally ill patients/families and their particular problems.
