Utah Home Health and Hospice is seeking a Clinical Nurse Coordinator to aid in our mission: to provide cost-efficient, quality home health and hospice services to the patients entrusted in our care. This role is specifically focused on home health and hospice services. As a Clinical Nurse Coordinator you will
- Assess, supervise and direct patient care
- Perform daily workflow duties on Homecare Homebase software
- Help our team maintain compliance with Medicare standards
- Perform patient care as required, including home assessments
- Ensure and chart compliance with state regulations
- Participate in community involvement including networking and marketing activities
- Evaluate and treat patients using the most current technology and practices
- Provide valuable input into the patient's care and treatment
