Utah Home Health and Hospice is seeking a Clinical Nurse Coordinator to aid in our mission: to provide cost-efficient, quality home health and hospice services to the patients entrusted in our care. This role is specifically focused on home health and hospice services. We are here to exceed the expectations of our patients, clients, and their families by providing excellent clinical care and premier service. *As a Clinical Nurse Coordinator you will:*

  • Assess, supervise and direct patient care
  • Perform daily workflow duties on Homecare Homebase software
  • Help our team maintain compliance with Medicare standards
  • Perform patient care as required, including home assessments
  • Ensure and chart compliance with state regulations
  • Participate in community involvement including networking and marketing activities
  • Evaluate and treat patients using the most current technology and practices
  • Provide valuable input into the patient's care and treatment

  • Licensed as a Registered Nurse by the State of Utah
  • *Home Health and/or Hospice Experience*
  • *Experience with Homecare Homebase is a huge plus, but not required*
  • Ability to communicate effectively, both verbally and in writing
  • Respect for patient privacy and strict confidentiality
  • Marketing or sales experience is a plus

Pay: $80,000.00 - $100,000.00 per year
