Utah Home Health and Hospice is seeking a Clinical Nurse Coordinator to aid in our mission: to provide cost-efficient, quality home health and hospice services to the patients entrusted in our care. This role is specifically focused on home health and hospice services. *As a Clinical Nurse Coordinator you will:*

- Assess, supervise and direct patient care
- Perform daily workflow duties on Homecare Homebase software
- Help our team maintain compliance with Medicare standards
- Perform patient care as required, including home assessments
- Ensure and chart compliance with state regulations
- Participate in community involvement including networking and marketing activities
- Evaluate and treat patients using the most current technology and practices
- Provide valuable input into the patient's care and treatment

- Licensed as a Registered Nurse by the State of Utah
- *Home Health and/or Hospice Experience*
- *Experience with Homecare Homebase is a huge plus, but not required*
- Ability to communicate effectively, both verbally and in writing
- Respect for patient privacy and strict confidentiality
- Marketing or sales experience is a plus
Pay: $80,000.00 - $100,000.00 per year
