What You'll Be Doing
Provide skilled home health nursing visits in patients' homes
Complete comprehensive assessments and ongoing evaluations
Perform wound care, medication management, injections, and disease monitoring
Develop and update individualized care plans
Educate patients and caregivers to improve health outcomes
Coordinate care with physicians and interdisciplinary team members
Document visits accurately in Homecare Homebase
Identify changes in patient condition and intervene appropriately
Who We're Looking For
Active Minnesota Registered Nurse (RN) license
Experience in home health, hospital, rehabilitation, skilled nursing, or acute care
Strong assessment, critical thinking, and clinical decision-making skills
Compassionate communicator who enjoys educating patients and families
Ability to work independently while managing a daily schedule
Reliable transportation and willingness to travel throughout the St. Paul service area
