Summary

As a dedicated and compassionate Licensed Practical Nurse (LPN), you will play a vital role in our nursing team at Torrington Center for Nursing and Rehabilitation. You will provide high-quality patient care, develop treatment plans, and work closely with our interdisciplinary team to ensure our residents receive the best possible care. Responsibilities

Provide direct patient care, including administering medications, wound care, and other medical treatments

Develop and implement individualized care plans in collaboration with the healthcare team

Monitor patient progress, identify areas for improvement, and make necessary adjustments

Communicate effectively with patients, families, and healthcare team members

Maintain accurate and up-to-date patient records

Participate in quality improvement initiatives and contribute to a positive and supportive work environment

Requirements

Current LPN license in the state of Connecticut

1 year of experience preferred (willing to train the right candidate)

Ability to work 8-hour shifts, including weekends and holidays

Strong communication and interpersonal skills

Ability to work effectively as part of a team
