About the Role

We are seeking a compassionate and skilled Registered Nurse (RN) to join our dynamic team at Woburn Rehabilitation and Nursing Center. As a Registered Nurse (RN) at our facility, you will be responsible for providing high-quality patient care to our residents, while also collaborating with our interdisciplinary team to create individualized care plans.

Responsibilities

Provide direct patient care to our residents, including administering medications, performing vital signs, and monitoring patients' conditions
Develop and implement individualized care plans, in collaboration with our interdisciplinary team
Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
Participate in quality improvement initiatives and contribute to our facility's commitment to excellence in patient care
Demonstrate a commitment to our facility's mission and values, and uphold our standards of excellence in patient care
