About this role

Registered Nurse (RN) – Home Health & Hospice. The RN will provide skilled nursing care to patients in their homes or hospice settings, ensuring high-quality clinical outcomes, patient safety, and comfort. Key responsibilities include conducting comprehensive patient assessments, developing individualized care plans, administering medications and IV therapies, providing wound care, monitoring patient progress, educating patients and families, coordinating care with interdisciplinary teams, maintaining EMR documentation, participating in on-call rotation, and providing compassionate end-of-life care.

Requirements

  • Current and valid Registered Nurse (RN) license in the State of Texas (or compact state)
  • Graduate of an accredited School of Nursing (BSN or ADN)
  • Current CPR (BLS) certification
  • Valid Driver's License, auto insurance, and reliable transportation
  • Minimum one (1) year of clinical nursing experience preferred
  • Home health or hospice experience strongly preferred
  • Knowledge of OASIS and EMR charting preferred
  • Knowledge of symptom management, palliative care, and end-of-life protocols
