Registered Nurse (RN) – Home Health Resource Visit Nurse at Home Health Care, Inc. Full-time role providing skilled home health nursing visits throughout multiple service areas in the Greater Twin Cities Metro Area. Responsibilities include performing comprehensive patient assessments, delivering wound care, medication management, injections and other skilled nursing services, educating patients and caregivers, collaborating with physicians and interdisciplinary care teams, floating between territories to support census growth and staffing needs, and completing documentation using Homecare Homebase (HCHB).

Requirements

Active Minnesota Registered Nurse (RN) license
Active Minnesota Registered Nurse (RN) license
Experience in home health, hospital, rehabilitation, skilled nursing, or acute care
Strong clinical judgment and assessment skills
Comfortable adapting to different territories and patient populations
Excellent communication and organizational abilities
Reliable transportation and willingness to travel throughout the Twin Cities Metro
