Responsibilities

  • Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility
  • Safely utilize equipment such as gait belts and Hoyer lifts
  • Monitor and record vital signs accurately and document care in EMR/EHR systems (PointClickCare preferred)
  • Provide care and support for patients with dementia, Alzheimer’s, and behavioral health conditions
  • Observe and report changes in patient condition to nursing staff
  • Maintain a safe, clean, and supportive environment for patients
  • Follow all infection control and safety protocols
  • Collaborate with nursing staff and interdisciplinary teams to ensure high-quality patient care

Qualifications

  • Active CNA certification in the state of Indiana
  • Previous experience in a skilled nursing facility (SNF), long-term care, or behavioral health setting preferred
  • Experience with PointClickCare or similar EMR systems preferred
  • Strong communication and interpersonal skills
  • Ability to work independently and adapt in fast-paced environments
  • Knowledge of HIPAA and patient confidentiality standards
