Hybrid Interim MDS Coordinator

This Full Time Hybrid Interim MDS Coordinator will provide coverage across multiple skilled nursing facilities in a hybrid capacity, combining on-site presence and remote work at assigned facilities. This role requires flexibility to travel between locations as needed and the ability to quickly assess each facility's current MDS processes, identify gaps, and implement corrective action as needed Essential Duties and Responsibilities: · Assesses and determines the health status and level of care of all new admissions. · Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change. · Communicates level of care for new residents to all disciplines. · Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident's stay. · Responsible for the data entry function to assure accurate data entry and electronic submission of MDS assessments. · Verifies electronic submissions of MDS, performs corrections when necessary and maintains appropriate records. · Interview residents and complete documentation in a timely manner based on scheduled assessments · Coordinates interdisciplinary participation in completing the Minimum Data Set (MDS) for all residents, including new admissions and ongoing assessments, in accordance with regulatory time frames; ensures completeness and thoroughness of documentation as mandated by federal, state, and medical standards. Physical Demands: Physical Demands · Required to sit, stand, bend and walk regularly; lift and/or move up to 25 pounds. · Visual and auditory ability are sufficient for written and verbal communication. · The noise level in the work environment is usually moderate Work Location: Hybrid — on-site at multiple skilled nursing facility locations across Central Illinois, combined with remote work (specific facility assignments, on-site schedule, and travel radius to be discussed during interview).
