Job Summary/Objective
Provides primary care nursing support, patient education reinforcement, health care maintenance, and patient care services to patients at Peoples Community Health Clinic in accordance with established protocols, provider orders, and RN/LPN scope of practice. Works with licensed providers and registered nurses to implement nursing care and established care plans. Observes, documents, and reports patient responses or changes in conditions.

Assists with the development, implementation, and reinforcement of health education programs and materials under the direction of licensed providers, registered nurses, or clinic leadership in accordance with established protocols and RN/LPN scope of practice. Provides day-to-day guidance and support to Patient Clinical Representatives within their defined roles, competencies, and responsibilities. Escalates patient needs, questions, or clinical situations that require licensed nursing assessment, interpretation, or independent judgement.
Performs all defined services and other related duties in accordance with the mission of Peoples Community Health Clinic.

Essential Functions

Provides primary care nursing support and patient care services within established protocols, provider orders, and LPN scope of practice. Reinforces patient education using approved materials, care plans, and direction from licensed providers or registered nurses. Provides patient counseling support only when protocol-driven, standardized, or directed by a licensed provider or registered nurse.
Observes, documents, and reports patient responses, concerns, changes in condition, or barriers to care to the appropriate licensed provider or registered nurse. Assists other care team members in coordinating patient care. Coordinates referral-related workflows, including initiating prior authorizations when required and within established procedures.
Supports a service-oriented atmosphere in accordance with Peoples Community Health Clinic’s mission and philosophy. Participates in efforts to improve work processes, clinical outcomes, health disparity initiatives, and quality improvement collaboratives. Follows organizational policies, procedures, clinical protocols, and applicable scope of practice requirements.
Develops and maintains clinical competency. Maintains a safe working environment and practices safe working habits. Assists in responsible use of Peoples Community Health Clinic resources.

This is an hourly position covered under the bargaining unit.
Expected Hours of Work
Typical work hours and days are between Monday and Friday, 8:00 a.m. to 5:00 p.m. and some flexibility in hours is allowed. Responsibilities occasionally may require irregular hours as deemed necessary. This may include an adjusted work schedule, long days, and early morning or late evening hours in order to meet client scheduling demands.

Experience
Graduate of an accredited nursing/practical nursing program. Relevant nursing experience preferred.
Licenses and Certificates
- Current Iowa LPN licensure
- Current BLS or CPR/First Aid certification
- Mandatory reporter training for child and dependent adult abuse certificate
