About this role
*About the Role* We are seeking a compassionate and experienced *Family Nurse Practitioner* to join our nationally recognized Patient-Centered Medical Home on a *part-time basis*. This role primarily focuses on *Chronic Care Management (CCM)* for adult patients with conditions such as diabetes, hypertension, and asthma, while supporting primary care services in an outpatient setting.
*Key Responsibilities*
- Manage and monitor care plans for patients with chronic medical conditions
- Perform patient evaluations, physical exams, and health assessments
- Diagnose and treat acute and chronic illnesses
- Order and interpret labs and diagnostic tests
- Educate patients on disease management and lifestyle modifications
- Collaborate with physicians and clinical staff to coordinate patient care
- Refer patients to specialists when appropriate
