Responsibilities

• Provide direct patient care to residents under RN supervision.
• Administer prescribed medications and treatments per physician orders; document administration.
• Monitor resident status, perform routine assessments, vital signs, and report changes promptly.
• Assist with activities of daily living and mobility; support rehabilitation as appropriate.
• Maintain accurate documentation, care plans, and communication with the interdisciplinary team.
• Respond to call lights promptly and maintain a safe, clean, and comfortable environment.

Requirements

• Active LPN license in Vermont (or eligible for licensure by endorsement).
• Current BLS (Basic Life Support) certification.
• Experience in long-term care or skilled nursing settings.
• Ability to work the scheduled night shift and weekends.
• Graduation from an approved practical nursing program.
• Strong clinical judgment and communication skills.
