Home Health Registered Nurse (RN)

Avida Home Health is seeking compassionate and motivated Registered Nurses (RNs) to join our growing team. Responsibilities

Provide skilled nursing care to patients in their homes
Complete comprehensive patient assessments and develop individualized care plans
Perform wound care, medication management, disease education, injections, catheter care, and other skilled nursing procedures
Monitor patient progress and communicate changes in condition to physicians and interdisciplinary team members
Accurately complete OASIS documentation and clinical charting
Educate patients and caregivers on disease management, medications, safety, and preventative care
Coordinate care with therapists, physicians, caregivers, and other healthcare professionals
Maintain compliance with Medicare, Medicaid, ACHC, and agency standards

Qualifications

Active Colorado RN license in good standing
Current BLS/CPR certification
Home health experience preferred, but not required
Strong clinical assessment and critical thinking skills
Excellent communication and organizational skills
Reliable transportation, valid driver’s license, and auto insurance
Ability and willingness to travel to patient homes throughout assigned territory
