Responsibilities
- Provide skilled nursing care to patients in their homes
- Complete comprehensive patient assessments and develop individualized care plans
- Perform wound care, medication management, disease education, injections, catheter care, and other skilled nursing procedures
- Monitor patient progress and communicate changes in condition to physicians and interdisciplinary team members
- Accurately complete OASIS documentation and clinical charting
- Educate patients and caregivers on disease management, medications, safety, and preventative care
- Coordinate care with therapists, physicians, caregivers, and other healthcare professionals
- Maintain compliance with Medicare, Medicaid, ACHC, and agency standards

Qualifications
- Active Colorado RN license in good standing
- Current BLS/CPR certification
- Home health experience preferred, but not required
- Strong clinical assessment and critical thinking skills
- Excellent communication and organizational skills
- Reliable transportation, valid driver’s license, and auto insurance
- Ability and willingness to travel to patient homes throughout assigned territory
