About this role
Regional Hospice is looking for a Full Time (40 hrs wkly) RN Case Manager for our Home Hospice patients in the community.

The Case Manager is responsible for the coordination and management of all patient care and other services for patients and their families. The Case Manager works with interdisciplinary team and the patient’s primary care physician to provide quality pain and symptom management, as well as support to the patient and family.

- Registered Nurse with a current license in the State of Connecticut REQUIRED
- Bachelor of Science in Nursing degree REQUIRED
- Minimum of 2 years' experience as a staff nurse in a hospital REQUIRED
- At least 1 year of experience in hospice preferably in home care REQUIRED
- Advanced credentials in hospice care preferred (CHPN).
- Transportation required, including proof of auto insurance and a clear driving history
- Excellent verbal, written and computer skills
- Excellent ability to communicate effectively with patients, families, physicians, interdisciplinary team members and others.
- Must possess good organizational skills and be able to work in a variety of environments.
- The RN Case Manager presents a professional attitude and appearance at all times while maintaining strict confidentiality of patient and agency information.
Job Type: Full-time Pay: $88,816.00 - $92,040.00 per year
