About this role

A Registered Nurse Navigator role aims to help facilitate the patient journey to ensure proper navigation throughout the health care system upon discharge disposition. The RN Navigator also aims at supporting staff to assist in the coordination of care while the patient is transitioning through the healthcare continuum throughout the outpatient healthcare setting. The RN Navigator will serve as the clinical resource to scheduling coordinators and assist in identifying appropriate service or provider for referrals. The RN Navigator will recommend alternative locations of care when appropriate and identify the discharge needs and services.

  • Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact
  • AHA BLS prior to end of orientation period
  • Two (2) years nursing experience required
  • Previous navigator work experience preferred
  • Background check
