Key Responsibilities Include Conducting comprehensive assessments, developing individualized care plans in collaboration with patients, families, and healthcare teams. Updating care plans based on patient condition changes and communicating effectively with team members. Providing direct patient care within the scope of practice, delegating tasks appropriately, and monitoring care quality.
