Position Summary

The GAFC RN Contractor plays a key role in evaluating participant needs, developing individualized care plans, and ensuring compliance with MassHealth GAFC program standards. Key Responsibilities Clinical Assessments & Home Visits

  • Conduct initial assessments , admission visits , and required 2‑week, 4‑week, and 60‑day reassessments .
  • Complete post‑hospital discharge visits , including the required 24‑hour RN visit .
  • Assess functional status, mental status, medication changes, home environment, and service needs.

Care Planning & Documentation

  • Develop and update Individualized Service Plans (ISPs) in collaboration with the Resident Service Coordinator (RSC).
  • Document all visits using outcome‑oriented nursing progress notes that meet MassHealth GAFC requirements.
  • Review and implement physician orders; complete all required reports in a timely manner.

Team Collaboration & Oversight

  • Provide clinical guidance to GAFC staff and caregivers.
  • Participate in interdisciplinary communication to support participant safety and well‑being.
  • Deliver staff training and in‑service education as needed.

Regulatory Compliance

  • Ensure all documentation and practices comply with EOEA and MassHealth GAFC regulations.
  • Maintain accurate, timely, and audit‑ready records.

QualificationsRequired

  • Active Massachusetts RN license
  • Current CPR certification
  • Strong computer proficiency (EMR/EHR systems, Microsoft Office)
  • Ability to work independently and travel to participant homes

Preferred

  • Bachelor’s degree in Nursing (BSN)
  • Experience in home health, case management, or MDS assessments

Language Skills (Preferred but Not Required)

Fluency in Spanish, or P ortuguese is highly valued.
