Position Summary
The GAFC RN Contractor plays a key role in evaluating participant needs, developing individualized care plans, and ensuring compliance with MassHealth GAFC program standards. Key Responsibilities Clinical Assessments & Home Visits
- Conduct initial assessments , admission visits , and required 2‑week, 4‑week, and 60‑day reassessments .
- Complete post‑hospital discharge visits , including the required 24‑hour RN visit .
- Assess functional status, mental status, medication changes, home environment, and service needs.
Care Planning & Documentation
- Develop and update Individualized Service Plans (ISPs) in collaboration with the Resident Service Coordinator (RSC).
- Document all visits using outcome‑oriented nursing progress notes that meet MassHealth GAFC requirements.
- Review and implement physician orders; complete all required reports in a timely manner.
Team Collaboration & Oversight
- Provide clinical guidance to GAFC staff and caregivers.
- Participate in interdisciplinary communication to support participant safety and well‑being.
- Deliver staff training and in‑service education as needed.
Regulatory Compliance
- Ensure all documentation and practices comply with EOEA and MassHealth GAFC regulations.
- Maintain accurate, timely, and audit‑ready records.
QualificationsRequired
- Active Massachusetts RN license
- Current CPR certification
- Strong computer proficiency (EMR/EHR systems, Microsoft Office)
- Ability to work independently and travel to participant homes
Preferred
- Bachelor’s degree in Nursing (BSN)
- Experience in home health, case management, or MDS assessments
Language Skills (Preferred but Not Required)
Fluency in Spanish, or P ortuguese is highly valued.
