About this role
In a collaborated effort with the Triage Team, the Oncall RN provides after hours hospice care to patients on an as needed basis. The Oncall RN may provide routine and/or emergent PRN visits. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides professional nursing care by utilizing all elements of nursing process. Assesses and evaluates patient's status, develops care plans, administers medications and treatments, counsels patient and family, acts as Case Manager when assigned, communicates with physicians and interdisciplinary group, supervises ancillary personnel, participates in quality assessment performance improvement teams and activities.
