Position Summary

The Hospice Community Liaison is responsible for developing and maintaining strong community and professional relationships that increase awareness of Peaceful Purpose Hospice and generate appropriate hospice referrals. This position serves as a clinical and community resource to hospitals, physicians, assisted living facilities, skilled nursing facilities, senior communities, case managers, social workers, home health agencies, and other referral partners.

The ideal candidate combines clinical knowledge with strong relationship-building, communication, and sales skills . Nursing experience is strongly preferred, with an LPN or RN being ideal. The Community Liaison must be comfortable discussing hospice eligibility, recognizing potential hospice candidates, educating referral sources and families, and working collaboratively with the clinical team to facilitate timely admissions.

Essential Duties and Responsibilities

The Community Liaison will develop and maintain referral relationships throughout the hospice service area and routinely visit hospitals, physician practices, assisted living facilities, skilled nursing facilities, senior communities, and other healthcare and community partners.

The liaison will educate referral sources regarding hospice philosophy, eligibility criteria, services, levels of care, disease progression, and the benefits of early hospice involvement. The liaison should possess enough clinical knowledge to recognize patients who may be appropriate for hospice and communicate relevant clinical information to the hospice clinical team for further evaluation.

The liaison will actively generate and follow up on referrals, maintain consistent communication with referral partners throughout the referral and admission process, and collaborate with nursing, intake, administration, social work, chaplain services, and other interdisciplinary team members to promote a smooth transition into hospice care.

The position will develop new referral relationships while maintaining existing accounts, identify opportunities for community outreach and education, participate in facility meetings, health fairs, senior events, professional networking opportunities, and community programs, and represent Peaceful Purpose Hospice professionally throughout the community.

The liaison will maintain an organized referral pipeline, document marketing activities and referral-source contacts, monitor referral trends, and provide leadership with regular updates regarding outreach activities, barriers, opportunities, admissions, and referral outcomes.

The liaison is expected to understand that successful hospice marketing is based on ethical relationship development and appropriate patient identification , not pressure-based sales. All activities must comply with Medicare hospice requirements, HIPAA, applicable state and federal regulations, and Peaceful Purpose Hospice policies.

Clinical Responsibilities

The Community Liaison should be capable of reviewing basic clinical information and recognizing common indicators of hospice appropriateness, including functional decline, recurrent hospitalizations, progressive weight loss, decreased oral intake, increasing dependence with ADLs, advanced dementia, metastatic cancer, end-stage cardiac or pulmonary disease, renal or hepatic decline, recurrent infections, and other indicators of terminal disease.

When clinically licensed and acting within the individual's scope of practice and assigned responsibilities, the liaison may assist with clinical assessments, obtaining clinical information, communication with physicians, or other clinical functions as authorized by Peaceful Purpose Hospice. Final hospice eligibility and certification remain the responsibility of the appropriate hospice physician and clinical team.

Preferred Qualifications

LPN or RN strongly preferred , with an active Georgia nursing license in good standing. Hospice, home health, hospital, long-term care, assisted living, case management, or other relevant clinical experience is highly desirable.

The ideal candidate will also have prior healthcare sales, marketing, business development, community liaison, admissions, or referral-development experience and a demonstrated ability to establish productive professional relationships.

The candidate should possess strong clinical judgment, excellent communication and presentation skills, confidence speaking with physicians and healthcare professionals, strong follow-through, organizational ability, and the ability to work independently in the community while remaining accountable to organizational goals.

A valid driver's license, reliable transportation, automobile insurance, and the ability to travel throughout the assigned service area are required.

Performance Expectations

Success in this position will be measured by consistent community presence, development and retention of quality referral relationships, appropriate referral generation, responsiveness to referral sources, effective follow-through from referral through admission, accurate communication with the clinical team, community education activities, and contribution to sustainable census growth.

The expectation is not simply to obtain referrals, but to build long-term relationships that make Peaceful Purpose Hospice a trusted hospice resource within the communities we serve.

Ideal Candidate

The strongest candidate is someone who can comfortably walk into a hospital case-management department and discuss a complex patient clinically, meet with an assisted-living administrator and identify opportunities to improve end-of-life support, educate a physician's office about hospice eligibility, sit with a family and explain hospice compassionately, and then follow the referral through until the patient receives the services they need.
