Overview Licensed Practical Nurse, Long Term Care, South Burlington, Vermont. Responsibilities

• Administer medications and monitor resident responses in accordance with care plans
• Assess and document residents’ conditions, reporting changes to the supervising clinician
• Assist with activities of daily living and mobility, promoting resident safety and comfort
• Maintain accurate and timely charting, including MARs and care notes
• Support wound care, treatment administration, and routine clinical procedures as directed
• Collaborate with interdisciplinary team to implement and adjust individualized care plans

Requirements

• Active LPN license (in good standing)
• Ability to work evening and/or overnight shifts
• Experience in or ability to adapt to a long-term care or skilled nursing environment
• Strong assessment, communication, and documentation skills
• Able to pass standard background check and any required screenings
• Comfortable working in a fast-paced, resident-centered setting
