- Provide direct nursing care, including wound care, medication administration
- Assist with patient education on disease management, medications, and lifestyle modifications.
- Monitor and document patients’ progress in accordance with care plans.
- Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care.
- Maintain accurate and timely documentation in *DSL*
- Ensure compliance with infection control protocols and healthcare regulations.
- Provide support for family and caregivers regarding patient care and treatment plans.
- Assist with mobility and activities of daily living (ADLs) as needed.
*Skills & Qualifications*
- *Valid California LVN license* and BLS certification required.
- 1 year of experience in *home health - *Required.
- Strong clinical skills in patient assessment, wound care, and medication administration.
- Familiarity with infection control practices and patient safety protocols.
- Ability to work independently while effectively communicating with the care team.
- Comfortable using electronic health records DSL
- Compassionate and patient-focused with excellent interpersonal skills.
