At Stillwater Hospice, we believe hospice care should feel deeply personal, compassionate, and supportive—for patients and for the clinicians who serve them. We are looking for an experienced PRN RN Case Manager who is passionate about providing meaningful end-of-life care and supporting families during an important time.
This is a flexible PRN role supporting our growing hospice team.
What You’ll Do
- Provide compassionate hospice nursing care to patients in their homes or care facilities
- Complete nursing assessments and develop individualized care plans
- Coordinate care with physicians, hospice team members, and family caregivers
- Provide education and emotional support to patients and families
- Document visits and care plans in the EMR system
- Participate in the interdisciplinary team to ensure high-quality patient care
What We’re Looking For
- Active Arizona RN license
- Hospice or home health experience preferred
- Strong clinical judgment and compassionate bedside manner
- Ability to work independently and manage patient visits
- Excellent communication and documentation skills
- Reliable transportation and valid driver’s license
