Responsibilities
As a Home Health RN, you will
Perform initial and ongoing comprehensive assessments of patients’ physical, functional, psychosocial, and environmental needs Develop, implement, and update individualized plans of care to ensure quality and continuity of patient care Determine patient eligibility and appropriate levels of care for admission and recertification in accordance with home health guidelines Provide skilled nursing interventions and initiate preventative and rehabilitative nursing procedures Educate
