About this role

Halcyon Home is seeking a dedicated and compassionate Hospice Registered Nurse Full Time to support our growing geriatric hospice population in Dallas, TX. Responsibilities include initial and ongoing comprehensive assessments, applying criteria for admission and re-certification to hospice care, implementing and documenting plans of care, providing effective communication to patients and families, applying infection control concepts, providing clinical directions to Hospice Aide and LPN/LVN, preparing clinical and progress notes, monitoring assigned cases for compliance, consulting with and educating patients and families regarding disease process and end-of-life care.
