Key Responsibilities
Coordinate patient care plans based on quality, timely service delivery, and medical necessity. Assess patient healthcare needs and determine appropriate services and levels of care. Develop, implement, monitor, and evaluate individualized care plans. Coordinate services with patients, families, healthcare providers, and support systems. Promote positive health outcomes through efficient, safe, and well-coordinated care. Support patients and families throughout the care process. Maintain respectful, approachable, and open communication with patients and care teams. Advocate for patient needs while preserving dignity, independence, and choice.
Qualifications
Required
Associate degree or higher. 1 year of healthcare experience.
1 year of experience in one or more of the following
Case Management Care Management EHR Coordination Active Registered Nurse (RN) license in Kansas. Strong communication, organizational, and interpersonal skills.
Preferred
Bachelor of Science in Nursing (BSN).
