Virtual Care Nurse

The Virtually Integrated Care (VIC) Medical Surgical Nurse works as the catalyst for patient centered care by leading a well-coordinated care team. The VIC RN is at the center of innovation and a transformational, evidence based practice care model. The experienced RN in this role will anticipate and direct the care of the patient through evaluating health promotion, risk reduction and clinical decision-making.

The Nurse assumes accountability for client care outcomes through the assimilation and application of evidence based practice to design, implement, and evaluate client plans of care in a virtual environment setting. This role functions as the center of coordination and communication for a multidisciplinary team. A high degree of professional competence and autonomy will be demonstrated in performing the roles of 1) clinician, 2) educator, 3) consultant/collaborator, 4) leader.

The VIC RN will function as a dynamic change leader for this transformational care model. The role is facility based for completion of orientation and has the potential to transition to a remote role with successful implementation of the care model.
