- Perform thorough patient assessments, including vital signs, physical examinations, and detailed health histories to inform individualized care plans.
- Ensure compliance with HIPAA regulations and infection control protocols while maintaining accurate medical documentation and medical records.
- Valid RN license with current registration in the state of practice.
- Proven experience working in acute care environments such as ADHC, Assisted Living and Skill Nursing settings.
- Proficiency in ICD coding (ICD-10/ICD-9) is preferred.
- Demonstrated expertise in specialized areas such as behavioral health is highly desirable.
- Experience working with diverse populations including, geriatrics (including memory care), individuals with disabilities or developmental delays.
- Knowledge of medical terminology, physiology principles related to patient assessment and diagnosis.
- Excellent communication skills for patient education and interdisciplinary collaboration; commitment to maintaining HIPAA standards.
- Critical thinking skills combined with physical stamina to handle demanding clinical situations efficiently.
- Ability to perform nursing assessments and reassessments
- Ability to develop and implement nursing portions of the Individual Plan of Care (IPC)
- Medication management and medication reconciliation
- Ability to recognize changes in participant condition and arrange appropriate medical follow-up
- Ability to work with the interdisciplinary team (physician, RD, PT, OT, SLP, LCSW, MSW, activity staff, etc.)
Pay: $33.00 - $35.00 per hour Work Location: In person
