  • Perform thorough patient assessments, including vital signs, physical examinations, and detailed health histories to inform individualized care plans.
  • Ensure compliance with HIPAA regulations and infection control protocols while maintaining accurate medical documentation and medical records.
  • Valid RN license with current registration in the state of practice.
  • Proven experience working in acute care environments such as ADHC, Assisted Living and Skill Nursing settings.
  • Proficiency in ICD coding (ICD-10/ICD-9) is preferred.
  • Demonstrated expertise in specialized areas such as behavioral health is highly desirable.
  • Experience working with diverse populations including, geriatrics (including memory care), individuals with disabilities or developmental delays.
  • Knowledge of medical terminology, physiology principles related to patient assessment and diagnosis.
  • Excellent communication skills for patient education and interdisciplinary collaboration; commitment to maintaining HIPAA standards.
  • Critical thinking skills combined with physical stamina to handle demanding clinical situations efficiently.
  • Ability to perform nursing assessments and reassessments
  • Ability to develop and implement nursing portions of the Individual Plan of Care (IPC)
  • Medication management and medication reconciliation
  • Ability to recognize changes in participant condition and arrange appropriate medical follow-up
  • Ability to work with the interdisciplinary team (physician, RD, PT, OT, SLP, LCSW, MSW, activity staff, etc.)

Pay: $33.00 - $35.00 per hour Work Location: In person
