We are seeking a dedicated and compassionate Registered Nurse to join our dynamic healthcare team. This position offers an exciting opportunity to make a meaningful difference in patients' lives while advancing your nursing career in a fast-paced environment.
Duties
Perform thorough patient assessments, including vital signs, physical examinations, and detailed health histories to inform individualized care plans.
Ensure compliance with HIPAA regulations and infection control protocols while maintaining accurate medical documentation and medical records.
Valid RN license with current registration in the state of practice.
Proven experience working in acute care environments such as ADHC, Assisted Living and Skill Nursing settings.
Proficiency in ICD coding (ICD-10/ICD-9) is preferred.
Demonstrated expertise in specialized areas such as behavioral health is highly desirable.
Experience working with diverse populations including, geriatrics (including memory care), individuals with disabilities or developmental delays.
Knowledge of medical terminology, physiology principles related to patient assessment and diagnosis.
Excellent communication skills for patient education and interdisciplinary collaboration; commitment to maintaining HIPAA standards.
Critical thinking skills combined with physical stamina to handle demanding clinical situations efficiently.
Ability to perform nursing assessments and reassessments
Ability to develop and implement nursing portions of the Individual Plan of Care (IPC)
Medication management and medication reconciliation
Ability to recognize changes in participant condition and arrange appropriate medical follow-up
Ability to work with the interdisciplinary team (physician, RD, PT, OT, SLP, LCSW, MSW, activity staff, etc.)
