About this role
TheraCare Home Health is seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality, patient-centered care in the home setting. Responsibilities include managing a caseload of patients, completing comprehensive assessments, coordinating care across the interdisciplinary team, performing Start of Care (SOC), Recertification, and Resumption of Care assessments, developing individualized plans of care, supervising LVNs and Home Health Aides, and providing patient and caregiver education.
