About this role
*Overview*
The per diem RN performs field-based clinical visits for an extended hours home care agency on an as-needed, visit-by-visit basis. The role centers on completing start-of-care (SOC) admission assessments in the client’s home, establishing the plan of care, and supervising and developing the home health aide/personal care aide workforce through supervisory visits, skills training, competency validation, and performance evaluations. The nurse works independently in the community with clinical oversight from the Director of Patient Services/Clinical Manager, and is responsible for ensuring care delivery meets physician orders, agency policy, and applicable state and federal regulations.

- *Conduct SOC/admission assessments: physical, functional/ADL, cognitive, safety and fall risk, skin, pain, medication reconciliation, and home environment.*
- Complete OASIS assessments (SOC, ROC, recert, transfer, discharge) where applicable and within required timeframes.
- Develop the plan of care and aide task sheet; obtain and clarify physician orders.
- Complete admission paperwork: consents, client rights, HIPAA, advance directives, emergency plan.
- Perform reassessments, change-in-condition, and discharge visits.
- Conduct supervisory visits at required intervals, observing aide care in the home.
- Train aides at point of care: transfers and body mechanics, personal care, toileting and catheter care, feeding, ROM, vitals, infection control, documentation.
- Complete skills validation and annual competency check-offs with return demonstration.
- Write aide performance evaluations; create and follow up on corrective action plans.
- Educate clients and families on disease process, medications, and safety.
- Coordinate with physicians, office staff, care managers, therapy, and DME vendors; obtain verbal orders.
- Investigate complaints, falls, hospitalizations, and incidents.
- Document in the agency EMR within required timeframes; maintain compliance with state regs, Medicare/Medicaid CoPs, HIPAA, OSHA, and accreditation standards.
*Qualifications*
- Current unencumbered RN license in state of practice; BSN preferred.
- 1–2 years clinical experience; home care, geriatrics, or med-surg preferred.
- OASIS experience/COS-C preferred; current BLS, physical, TB screening.
- Valid driver’s license, reliable transportation, and auto insurance.
- EMR proficiency
- Excellent communication skills for effective patient assessment, education, and interdisciplinary collaboration; ability to adapt quickly to fast-paced environments such as emergency rooms or urgent care settings.
