About this role
The hospice case manager is responsible for coordinating and managing the care of hospice patients to ensure comfort, dignity, and quality of life. This role works closely with patients, families, physicians, and the interdisciplinary care team to develop and implement individualized care plans, monitor patient needs, provide education and support, coordinate services, and ensure compliance with hospice regulations while delivering compassionate end-of-life care.
