At Avir at Santa Fe, we are seeking a compassionate and skilled Registered Nurse or Licensed Vocational Nurse to join our dedicated healthcare team. As a premier skilled nursing facility, we provide personalized care to our residents from short-term rehabilitation to long-term care. Responsibilities
Provide high-quality, patient-centered care to our residents, including administering medications, performing treatments, and implementing care plans
Collaborate with interdisciplinary teams to develop and implement comprehensive care plans that meet the unique needs of each resident
Monitor and report changes in residents' conditions to ensure timely interventions and optimal outcomes
Maintain accurate and detailed records of residents' care, treatments, and medications
Participate in quality improvement initiatives to optimize patient outcomes and improve our care delivery processes
