About this role
American Hospice and Home Health Services is currently seeking a Full Time, Part Time, and/or Per Diem LVN to service Santa Clara County. The LVN will provide nursing care and supportive services in accordance with the physician/provider treatment plan and the nursing process of assessment, diagnosis, planning, implementation and evaluation under the supervision of the Registered Nurse. Performs accurate assessments, interprets and correlates findings, establishes plan of care, observes patient progress, records pertinent observation, implements physician and nursing treatment plans and evaluates outcomes.

Requirements
- LVN
- Licensed Vocational Nurse License in California
- Preferred 1-2 years Professional Experience in Home Health and Hospice
- Current CPR Certification
- Home Health experience preferred
- IV Certification
