About the role

Conexiones ATS/CSS is a 32-bed facility that provides clinical stabilization services to adults struggling with drugs, including heroin and prescription drugs, or alcohol. Acute treatment services designated for up to 12 individuals for medically managed withdrawal services. At Casa Esperanza Conexiones, located at the Tewksbury Hospital, we provide residential substance abuse treatment for adults in a physically and psychologically safe environment and additional inpatient medically managed detoxification services, including those with co-occurring mental health issues.

The program is staffed by highly skilled professionals who work with each patient to develop an individualized treatment plan and determine appropriate aftercare. We are currently seeking a Licensed Practical Nurse at our Tewksbury Conexiones through Casa Esperanza’s Integrated Care Model, implementing doctor’s orders, monitoring specified medication/refills, monitoring and recording vital signs as needed, and maintaining medical/ nursing records. The Nurse will also observe patients and provide accurate and timely documentation of their treatment progress, and coordinate and assist Case Managers regarding client’s progress and treatment.

What you'll do

Complete nursing assessments, COWS, and CIWA assessments upon admission Complete COWA/CIWA and ASAM assessmentss aligned with medical orders for protocols associated to withdrawal management. Complete medical clearance exam at time of intake Complete medication monitoring and all required documentation Complete medication counts at the start and finish of the shift of all control and non-control substances Work closely with the clinical team during MDT meetings to offer and provide client medical insight as part of case presentations. Consult with staff and attend case conferences, as needed Work collaboratively with the medical director for the medication reconciliation process.

Works to achieve standing orders and medical orders for medication. Able to organize and keep record of frequency and urgent medical exams Directly works in collaboration with community providers for medication refills and reconciliation. Complete evaluations based on client request for medical attention, emergency and non-emergency triage and physical/mental health rounds for safety and condition monitoring Keep electronic health records up to date including progress notes and assessments of service.

Educating clients on their current medical diagnosis, needs and medical histories; educating clients on medication information and usages Alerting physicians when conditions worsen or if abnormalities arise. Works in collaboration with counselors to ensure proper medication management and medication planning Works directly with the program director on any medication errors and medication monitoring procedure Works uniformly with the recovery specialist on the medication monitoring procedure Ensure responsibilities regarding Duty to Warn and Mandated Reporting are fulfilled

Full-time, Monday-Friday from 11 pm to 7 am (3rd shift)

Hourly Rate

$40 Base Rate

2nd shift: +$2/hr

Overnight Shift: +$3/hr

The pay range for this role is
