We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our dynamic healthcare team.   • Conduct comprehensive patient assessments, including vital signs, physiology evaluation, and medical history review, to determine individualized care plans.
  • Manage inpatient and outpatient cases across diverse specialties such as critical care, trauma, emergency medicine, pediatrics, geriatrics, behavioral health, and medical-surgical units.
  • Coordinate discharge planning by collaborating with interdisciplinary teams to ensure timely and appropriate post-acute care arrangements.
  • Oversee utilization management processes by reviewing diagnostic evaluations, medical records, and hospital documentation to optimize resource use.
  • Facilitate communication with patients, families, healthcare providers, and managed care organizations to ensure clear understanding of treatment plans and care objectives.
  • Maintain accurate documentation of patient information in medical records while adhering to HIPAA regulations for patient privacy and confidentiality.
  • Support intake procedures by evaluating patient needs in both inpatient and outpatient settings, including clinic visits and emergency room encounters. Requirements
  • Valid nursing license with current registration in the relevant state or jurisdiction.
  • Proven experience in critical care environments such as ICU or Level II/Level I Trauma Centers; experience in emergency room or hospital medicine is highly desirable.
  • Strong background in case management within acute care settings, including post-acute care coordination and discharge planning.
  • Knowledge of medical terminology, physiology, diagnostic evaluation methods, and inpatient/outpatient clinical workflows.
  • Familiarity with managed care processes, utilization management protocols, and medical records documentation standards.
  • Excellent patient assessment skills coupled with the ability to interpret complex clinical data effectively.
  • Experience working with diverse populations including pediatrics (PICU), geriatrics, behavioral health patients, and those requiring infectious disease care.
  • Ability to navigate hospital environments confidently—hospital experience across various departments such as OR (Operating Room) experience is a plus.
  • Strong communication skills with an emphasis on patient service and multidisciplinary collaboration.
