*Pay:* Up to *$38/hour (DOE)* *Bonus:* Up to *$500/month*
First Call Home Health is a locally owned and operated agency providing skilled in-home services throughout the Willamette Valley. We are seeking a *Home Health LPN* to join our compassionate and supportive team.

The LPN provides skilled nursing care to patients in their homes under the direction of an RN Case Manager. This role focuses on patient assessments, treatment implementation, documentation, and patient/family education.
- *Full-time strongly preferred*
- Assigned territory based on location closest to your home
- Average *5–7 visits per day*
- Caseload of approximately *25–30 patients*
*Key Responsibilities*
- Perform skilled nursing visits within LPN scope of practice
- Follow established plans of care
- Document patient care accurately and timely
- Communicate patient progress to the care team
- Promote patient safety, infection control, and education
- Maintain confidentiality and professional standards
*Qualifications*
- Current *LPN or LVN license*
- Valid driver’s license & auto insurance
- Current CPR certification
- 1 year of LPN experience preferred
- Home health experience preferred
- Ability to lift up to *50 lbs*
