- Conduct thorough patient assessments to determine eligibility for hospice services and develop individualized care plans aligned with patient needs and wishes.

- Manage a caseload of hospice patients, coordinating medical, emotional, and psychosocial support in collaboration with physicians, social workers, chaplains, and other healthcare professionals.
- Monitor patient progress through regular evaluations, vital signs monitoring, and symptom management to ensure comfort and quality of life.
- Educate patients and families about disease progression, medication management, advanced directives, and available resources to empower informed decision-making.
- Oversee discharge planning, transition of care, and coordination with home health agencies or inpatient facilities as needed.
- Maintain accurate electronic health records (EHR) including documentation of patient interactions, clinical notes, medication administration, and compliance with HIPAA regulations.
- Participate in interdisciplinary team meetings to review patient status, adjust care plans, and ensure seamless delivery of hospice services.
Qualifications
- Valid nursing license with current registration in the state of practice.
- Proven experience in critical care settings such as ICU, emergency medicine, trauma centers (Level I or II), or medical-surgical units.
- Prior experience in hospice or palliative care is highly preferred; home health or post-acute care experience is a plus.
- Strong knowledge of physiology, symptom management, infectious disease care, discharge planning, and medical terminology.
- Familiarity with electronic health records (EHR/EHR systems) and healthcare regulations including HIPAA compliance.
- Excellent patient assessment skills combined with the ability to collaborate effectively with multidisciplinary teams.
- Ability to manage complex cases involving behavioral health issues or geriatrics; experience with managed care policies is advantageous.
- Exceptional communication skills for client interaction and family education across diverse populations.
- Knowledge of hospital medicine practices including inpatient experience is beneficial; outpatient or clinic experience is also valued.
