Join a Team Where You Truly Belong: Iowa City Hospice is adding a Hospice Nurse Case Manager (RN) to our team. As a Hospice Case Manager (RN), you will coordinate and deliver individualized, patient-centered care in homes and care facilities. You'll build meaningful relationships with patients and families while working both independently and collaboratively with an interdisciplinary team. In this role, you will play a vital part in supporting comfort, dignity, and quality of life, while guiding families through the caregiving journey with compassion and expertise.
