Licensed Practical Nurse

Advantage Care Solutions is a home health agency currently seeking per diem LPNs to join our team. We treat health issues before they become more complex in the comfort of the patients' homes. Our ideal candidate is a licensed LPN with a minimum of one year of clinical experience, preferably in home health or a related field. Duties

• Administer medications and treatments, provide wound care, IV Infusion, colostomy care, urinary catheter care, and other orders as prescribed by physicians- Assist in discharge planning and provide patient education on post-discharge care- Monitor and record vital signs, symptoms, and changes in patient condition- Collaborate with healthcare team members to develop and implement individualized care plans- Use electronic medical record systems such as Kinnser to document patient information accurately

Qualifications

• Valid LPN license in the state of Florida- Minimum of one year of experience in a healthcare setting, preferably with experience working in Home Health- Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members- Ability to prioritize tasks and work efficiently in a fast-paced environment- Excellent attention to detail and ability to follow instructions accurately
