Position Overview
We are seeking a compassionate, organized, and self-motivated Registered Nurse (RN) to join our home health team. The RN will provide skilled nursing care in the home setting, manage an assigned caseload, and coordinate care with the interdisciplinary team.
Key Responsibilities
- Provide skilled nursing care to patients in their homes
- Manage a patient caseload and coordinate individualized care plans
- Supervise care provided by LPNs in accordance with clinical and regulatory standards
- Complete all documentation in Axxess Home Health EMR, with end-of-day charting required
- Collaborate with physicians, therapists, and other care team members
- Educate patients and families on disease management, medications, and home care procedures
- Participate in rotating on-call schedule for after-hours coverage
Qualifications
- Active RN license in the state of Kansas
- 1+ years of nursing experience preferred (home health experience a plus)
- Proficient with EMR systems (Axxess experience a plus)
- Strong clinical, organizational, and time-management skills
- Valid driver's license and reliable transportation
- Independent, compassionate, and team-oriented
Job Type: Full-time Pay: $42.00 - $44.00 per hour
