What you'll need
Active LPN or RN License issued by NYS Minimum of 6 months experience in a home care setting. Current CPR/BLS Certification Empathy, Reliability , and a commitment to high-quality care
What you'll do
Perform 1:1 patient care duties and treatments as per care plan. Provide health promotion, counseling, and education. Administer medications and other personalized interventions.
Coordinate care, in collaboration with a wide array of health care professionals. Document nursing notes regarding patient care daily. Must be able to lift 50lbs.
