We’re seeking a compassionate and motivated Clinical Liaison RN to join our hospice team. This role blends clinical expertise with relationship-building, helping patients and families understand hospice services and ensuring smooth transitions into care.
- Key Responsibilities
- Complete hospice eligibility assessments in hospitals, SNFs, and community settings
- Educate patients, families, and referral partners on hospice philosophy and services
- Coordinate admissions and ensure timely, accurate documentation
- Build strong relationships with physicians, case managers, and discharge planners
- Serve as a clinical resource to referral sources and internal teams
