Responsibilities

  • Administer prescribed medications and basic treatments under RN supervision and facility protocols.
  • Monitor resident health status, document changes, and report concerns to the nursing team.
  • Perform routine nursing

vital signs, wound care, daily living assistance, and basic wound/skin care as directed.

  • Support activities of daily living and provide compassionate, patient-centered care to residents.
  • Maintain accurate charting and ensure compliance with care plans, safety, and infection control practices.
  • Collaborate with interdisciplinary team to coordinate care and respond to resident or family questions.

Requirements

  • Active Licensed Practical Nurse license in good standing (state of NH or eligible to practice in NH)
  • Graduation from an accredited practical nursing program
  • Ability to work the night shift (10:45 p.m. – 7:15 a.m.)
  • Strong communication and documentation skills; comfortable with basic charting
  • Willingness to complete required orientation and comply with facility policies and procedures
