Responsibilities
- Administer prescribed medications and basic treatments under RN supervision and facility protocols.
- Monitor resident health status, document changes, and report concerns to the nursing team.
- Perform routine nursing
duties
vital signs, wound care, daily living assistance, and basic wound/skin care as directed.
- Support activities of daily living and provide compassionate, patient-centered care to residents.
- Maintain accurate charting and ensure compliance with care plans, safety, and infection control practices.
- Collaborate with interdisciplinary team to coordinate care and respond to resident or family questions.
Requirements
- Active Licensed Practical Nurse license in good standing (state of NH or eligible to practice in NH)
- Graduation from an accredited practical nursing program
- Ability to work the night shift (10:45 p.m. – 7:15 a.m.)
- Strong communication and documentation skills; comfortable with basic charting
- Willingness to complete required orientation and comply with facility policies and procedures
