Our team of healthcare professionals, including therapists, aides, and medical social workers bring our services into the comfort of our client's home. We provide high-quality, competent care delivery, patient advocacy, a respectful and nurturing work environment, and community healthcare engagement. We are excited to welcome an experienced Registered Nurse -Skilled Home Health who embodies our values. The ideal candidate will:

• Provide professional services within the scope of nursing practice standards,
• Collaborate with primary care physicians, and
• Teach patients and their families.

Responsibilities will include

• Providing services to our clients using substantial and specialized nursing skills,
• Initiating appropriate preventative and rehab nursing procedures,
• Preparing and submitting timely clinical and progress notes,
• Overseeing coordination of patient services,
• Completing medical history/home safety check/environmental assessments,
• Performing socio-psychological evaluations,
• Developing an individualized Plan of Care (POC) to be submitted to the physician for approval,
• Determining medical necessity for other services,
• Evaluating the patient’s ADL and IADL abilities and need for a home health aide,
• Supervises HHA services and associated requirements and documentation,
• Review billing processes, including co-pay requirements, and
• Exceptional communications with clients, co-workers, and the general public.
• Quality Assurance Experience (QA)

Qualifications

• Graduate of an approved professional nursing program (RN) (Required)
• Licensed “in good standing” in North Dakota & Minnesota (Preferred)
• Experience in the home healthcare industry (Required)
• Organized and meticulous with an exceptional level of confidentiality and attention to detail
• Clean criminal background check and drug screening
• CPR certification
