Licensed Practical Nurse

As patients come onto service, our Staffing Team will contact you when coverage is needed. You choose how much you want to work. Whether you want to see several patients a day or just a few visits each week, you have the flexibility to set your own pace.

Flexible scheduling – You coordinate visit times directly with your patients. Choose your workload – Take on as many or as few visits as fit your schedule. No assigned visit times – You arrange visits based on what works best for you and your patients. Typical visits are 45–60 minutes. Travel included within your established territory. Additional compensation may be available for patients outside your normal territory on a case-by-case basis. Work independently while having the support of our clinical and staffing teams. Gain experience in Home Health with support from an experienced team.

What You'll Do

Provide skilled nursing care to patients in their homes or residential facilities. Assess patient needs and communicate changes in condition to the RN Case Manager and Director of Nursing. Follow established plans of care and provide appropriate nursing interventions. Monitor and document patient progress and care provided. Build meaningful relationships with patients and their families. Deliver both excellent clinical care and a compassionate patient experience. Travel to patient locations throughout your assigned service area.

Qualifications

Required

• Active LPN license in the state of practice
• At least 1 year of Home Health experience
• Valid Driver's License
• Valid Auto Insurance
• CPR or BLS Certification
• AZ Level One Fingerprint Clearance Card (Arizona applicants only)
• Physical/medical clearance to perform job duties (Nevada applicants only)
• Valid TB Test — skin test within the last year or chest X-ray within the last five years
