Registered Nurse – Part Time, Adult Day Health

• 24 hours per week

Key Responsibilities

The RN will

• Provide nursing oversight and supervise the delivery of nursing care to ADH participants.

• Complete initial and ongoing nursing assessments and evaluations.

• Develop, implement, and monitor individualized comprehensive care plans.

• Monitor changes in participants’ health and communicate with physicians, families, care coordinators, and other appropriate providers.

• Review and maintain accurate clinical documentation and participant records in accordance with program and funding requirements.

• Coordinate care with physical, occupational, and speech therapy providers and other community service providers.

• Participate in discharge planning and coordination of services.

• Monitor vital signs, symptoms, medication compliance, weights, and other health indicators.

• Assist staff during medical emergencies.

• Maintain positive relationships with participants, families, subcontractors, and community partners.

• Participate in required staff meetings, training, and professional development.

Qualifications

Required

• Current Georgia Registered Nurse (RN) license

• At least one year of clinical experience in healthcare or social services

• Current First Aid/CPR certification

• Successful completion of required background check, drug screening, and physical

Preferred

• Experience working with older adults

• Experience working with individuals with dementia or cognitive impairment

• Experience with chronic health conditions

• Experience in adult day health, home health, long-term care, or community-based healthcare
